Gerould's Professional Pharmacies and Healthcare Centers have developed a notice of privacy, a requirement of NYS and the federal HIPAA regulations. This policy is to provide you, the patient, with a description of how we use your protected health information within our professional practice. The following will disclose all uses of the information as it relates to urchasing and billing medical equipment and related supplies.

Purchasing: When an item requires a prescription before purchasing, and/or the item will be billed to an insurance company or credit card, certain information is required. Any missing information or concerns about the equipment, may require a phone call to the prescriber's office requiring communication of patient information. This information is stored in a secure computerized system and files that are accessible only to staff.

Billing: The communication of patient information is limited to purposes of billing equipment or supplies to a third party insurance company, medicare and medicaid or a credit card company. This may require that patient information be relayed, as well as, type of equipment and diagnosis. This information is communicated by phone, fax and computer.

Storage and Disclosure: Gerould's uses a secure, state of the art computerized systme. All information is restricted to users by password and cannot be accessed by external parties without the authorization from the staff.

Shipping Policy: Shipping charges that are shown may be approximate and will be adjusted according to destination. Local delivery, Chemung County in NY and surrounding areas, may not have a shipping charge.
